MOPR Notes:  Ref LEV20051121 where DC explains this, his written list of items to be resolved before VP gets finalization on the levee.  Three X's (XXX) below identify Items that he had lined out.

 

PUNCH LIST FOR ITEM IV-B:  Fishpot Structure - JB-1 Area

 
1.          Steel pipes filled with concrete (bollards) need to be placed on the north side of junction box to prevent any vehicles from jumping curbs & hitting operators & junction box.  # & location of bollards to be determined in field.
2. XXX One section of fiberglass grating on the south side of structure is wobbly.  Concrete needs to be ground off level so grating sits flat on top of junction box.
3. XXX Arrows need to be placed on operators to indicate direction to "Open" or "Close" sluice gate (see Part 2.5.7 Page 15100-7)
4. XXX Anchor bolt nuts should be bronze (see Part 2.5.7 Page 15100-7)
5. XXX Repair erosion & washout areas.
6. XXX Reseed, mulch & fertilize as required.

PUNCH LIST FOR ITEM IV-B:  Route 141 Gatewell & Outlet - GW1

 

1. XXX

Anchor bolt nut should be bronze (see Part 2.5.7 Page 15100-7)
2. XXX Arrows need to be placed on operator to indicate direction to "Open" or "Close" sluice gate (see Part 2.5.7 Page 15100-7)
3.         Gravel road on crown of levee needs to be extended from the emerg road to beyond 141 Gatewell.  (so the emerg equipmt vehicle has access to gatewell operator)
4.         Telephone & cable lines need to be raised.
5. XXX Emerg road gate needs to ((be)) relocated to allow fire truck to make turn onto emerg road from MerStaRd.  Distance between gate post needs to be A {sic} wider.  (locations & distances to be determined in field)  (Ramp Road A)
6. XXX Sheet pile to be cut off & fill placed.
7.         Repair erosion & washout areas.
8.         Reseed, mulch & fertilize as required.

Pkg on gravel area between flood wall & blacktop.  Some type of barrier needs to be installed to prevent damage to flood wall if used for pkg & storage of vehicles.

PUNCH LIST FOR ITEM IV-B:  Detention Area B

 

1.     

A storm water pipe needs to be installed under Ramp Road B to drain the area between the BNSF RR Right Way & Ramp Road B & the landside of levee slope southwest of Ramp Road B into SAI-B.
2.     

Repair erosion & washout areas.
3.     

Reseed, mulch & fertilize as required.
4.      City to investigate storm water drain east side of 3rd St & advise COE.
5.      Storm water pipe between EXI-B & FES on the east side of the Ramp Road B needs to be changed so this pipe will drain properly.

1.         Clean River Dr from Ramp Road A (west of Hwy 141) to Ramp Road B (east of BNSF RR Bridge (( ) )) & Repair.
2.         Perman((en))tly block River Dr on east side of Ramp Road B.
3.         Clean & repair River Dr from Kena Ave west to the west entrance of the Boat Ramp Pkg Lot.
4.         Erosion on river & land sides of levee needs to be repaired, reseeded, mulched & fertilized as required.
5.         Natural gas line over levee at Pharoah Ave needs to be inspected by COE & approved that it meets all requiremts of specs.
6. XXX Security gate on River Dr west of Kena Ave to be moved & installed at the sourth end of Pharoah Ave (City VP will relocate gate if COE approves)
6.         Silt fences need to be repaired on river & land-side of levee.

PUNCH LIST ITEM For St Louis Ave Closure Structure

 
 

Sill plate across St L Ave is loose.  Dwayne Actkley ((should be Duane Atchley)) COE Structural Eng has worked up a plan to repair this problem.  (COE responsible for repair)

 

PUNCH LIST ITEMS For  III-B:   Grand Glaize Creek ((GG Creek))

 
1. Storm water pipe on north side of Leonard Pk at toe of land-side levee continues to wash out (city has filled washouts a couple times).  COE rep inspected inlets & pipe joints & suggest to city worker & myself a way to test for leakage.  City workers tested pipe & inlets & found sufficient signs of leakage & needs extensive work to repair.  (City is obtaining a cost est to make repairs.  If city makes these repairs they will expect credit from their share.)
2. Fiberglass grating on junction box at the GG Det Pond & inlet to sluice gate is to((o)) weak an((d)) needs to ((be)) replaced with galvanized steel grating.  (City has placed steel plates over grating to prevent someone from falling into junction box structure.)

PUNCH LIST ITEMS For III-B:   Grand Glaize Creek ((GG Creek)) ((Cont'd))

 
3. Dike between north side of the creek channel & south side of wetland pool continues to erode away.  COE has repaired the dike on the north side of the creek channel.  Some of this repair looks good but there is a large section that is severely eroding.  COE needs to inspect the damage & access if dike washes out will it cause damage to creek-side of levee. (({sic} sentence structure))

 

PUNCH LIST For ITEM  II:   Fishpot Creek

 
1. Sill plate across Vance Rd is loose.  Dwayne Actkley ((should be Duane Atchley)) COE Structural Eng has worked up a plan to repair this problem.  (COE responsible for repair)
2. The sluice gate operator needs a new stem cover (Cover has been replaced twice.  Vandals have removed it.)  Can city crew make new one?  (may not match original)
